Residential Cost Report Reminders

Residential Cost Reports for 2017 are due by March 31, 2018.  Reminders:

  • The 2017 Cost Report templates and resource materials are available on the DDA Residential Provider Resources page.
  • A Residential Staffing Survey is included under tab "O," titled "Schedule O."
  • Complete a separate survey for each program/contract.
  • Data must be separated by Supported Living or Group Home.
  • Requests for extensions beyond the March 31, 2018 deadline must be submitted to the Regional Administrator by March 15, 2018.
