The new tool – My Page –a document created to help our clients with developmental disabilities communicate with their case resource managers, is now available for use.

My Page is a one-page profile for clients to complete about their personal wants, desires and goals to get the most out of their person-centered assessments. Information supplied in the My Page profile can result in clearer, and more personalized discussions between clients and their case resource managers.

Using My Page is not mandatory, but it is strongly encouraged. My Page has the ability to help engage conversation, save time and enhance the overall person-centered assessment process.
