Dear Residential Provider,

Due to changes in reporting requirements, and in accordance with WAC 388-829, we simplified the form used for requesting reimbursement to the contractor for the trainer delivering the Residential Core 40 hour training to Direct Support Professionals.  

The reimbursement form and staff roster is now combined into one document and reflects the updated reimbursement amount of $210 per student.  You can access the new Residential Training Roster/Reimbursement form on our website.

When requesting reimbursement submit the new form AND training evaluations to the assigned Resource Manager.  If scanning and sending electronically, please attach the Residential Training Roster/Reimbursement form and evaluations as separate documents.
