The Washington State Adult Vaccine Program (AVP) provides vaccine for adults 19 years of age and older who are uninsured.

If you are a provider who was enrolled in AVP last year:
AVP re-enrollment emails were sent out to facility contacts (primary/ back-up vaccine coordinators and signatory on file) on June 1, 2026. These emails contained a link to each facility's pre-populated, facility specific provider agreement to update and sign.
- Most of your information from last year’s provider agreement will be saved, so you will only need to update information that has changed or is out of date.

If you are a provider who was NOT enrolled in AVP last year:
- Please use this to walk you through the process.
- To enroll, facilities will complete the Adult Vaccine Program Provider Application . The Washington State Department of Health will review your facility’s information and email the submitter a link to fill out the Adult Vaccine Program’s full provider agreement. Receiving the link may take up to 72 hours. Once the link is received, facilities may proceed with filling out the AVP provider agreement.
The enrollment deadline for providers who wish to participate in the upcoming order cycle this fall is July 17, 2026. We encourage providers to submit early.
 The Childhood and Adult Vaccine Program teams at the Department of Health are currently operating with reduced staffing and appreciate your patience as we work through your questions and requests. Please allow us 72 hours to respond to non-urgent inquiries. Sending multiple emails about the same issue may delay response times. We encourage providers to use a single email thread whenever possible and please be sure to include your PIN.
