If you are a provider who was enrolled in AVP last year who wishes to re-enroll:
- Your facility contacts (primary/ back-up vaccine coordinators and signatory on file) will receive an automated email on June 1, 2026 with a link to your pre-populated, facility specific provider agreement to update and sign. Do not use the link on the AVP website to submit an enrollment inquiry if you are re-enrolling, as this will result in delays.
- We will send out an AVP Newsletter when re-enrollment links have officially been sent out.
- Most of your information from last year’s provider agreement will be saved, so you will only need to update information that has changed or is out of date.
- If you need to update your facility contacts, please reach out to us at WAAdultVaccines@doh.wa.gov.
If you are a provider who was NOT enrolled in AVP last year:
- Please use this Adult Vaccine Program Enrollment Guide to walk you through the process.
- You may use the link below starting on June 1, 2026 to begin enrolling in AVP. Please do not start enrolling prior to June 1 or you will need to complete the enrollment form again.
The enrollment deadline for providers who wish to participate in the upcoming order cycle this fall is July 17, 2026.
 You can gather important information now to save you time when enrollment opens. Here are some important items you may want to compile now:

Vaccine Selection: The enrollment form will prompt you to select up to 5 vaccine types that you would like the option to request this year through AVP. Please consider these options carefully as you will not have the option to change your selections until the next enrollment period in 2026.

Training requirements: Ensure that the annual DOH- specific vaccine coordinator training requirements are completed and that you have saved your course certificate to upload with your agreement.

Shipping Details: Verify the days of the week and core business hours clinic staff are available to receive vaccine deliveries.
- The facility must be open to receive vaccine deliveries two days a week, Monday through Friday, for a minimum of four consecutive hours per day.

Cold Storage Unit Details: Verify information on all units storing publicly supplied vaccine.
- Cold Storage Units: unit type, manufacturer, effective date, and purchase/issue date.
- Thermometers: thermometer type, make and model of thermometer (entered in serial number field), date of last calibration, and calibration expiration date.
- A photo for proof of brand/model, capacity and calibration certificate is required for each unit listed.

Practicing Providers: List all licensed health care practitioners (MD, DO, NP, PA, pharmacist, etc.) at your facility who have prescribing authority for AVP vaccines.

Patient Population: Report the approximate number of uninsured patients (19+ years of age) served by your facility in the last 12 months. If you did not see any uninsured adult patients, please indicate the number of patients you anticipate you will serve in the coming 12 months.

Keep in mind:
- Your AVP order set will only contain the vaccine products you select in this section.
- Your ranking will help us prioritize your preferred vaccine products during the AVP allocation process in the event we cannot fill all requests due to funding limitations.
- You are not guaranteed to receive an allocation of the vaccine types selected.
- Leave selections blank if you wish to request less than 5 vaccine types.

You must select COVID-19 vaccine as one of your top 5 priorities if you wish to request COVID-19 vaccine through AVP this fall. AVP will no longer have additional funding specifically for COVID-19 vaccine. All vaccines, including COVID-19, will be purchased using our annual 317 funding. More information will follow regarding which COVID-19 vaccine products will be available to order through the program.
- Do not select more than 1 brand of each vaccine type in your top 5. For example, do not prioritize 2 Hepatitis A products (i.e. Vaqta & Havrix) or 2 Tdap products (i.e. Adacel and Boostrix).
 Primary and back-up vaccine coordinators must complete this annual training requirement within the current calendar year, prior to their provider agreement renewal deadline. It is also best practice to have all staff who handle and administer vaccines complete the training as well. The training is a nine-module, web-based, immunization training course that presents program-specific immunization content in a step-by-step, self-study format.

**During enrollment and re-enrollment, providers are required to upload a certificate of completion for the blended learning series, which can be downloaded from Train.org. Uploading a continuing education certificate, individual module certificate, or any other document will result in delays and the agreement being returned for edits.

Continuing education credits for the AVP Vaccine Coordinator Training will be temporarily unavailable for pharmacists and pharmacy technicians during June and July while we close out the state fiscal year.
The Final Assessment and Evaluation module will be reactivated in August, and availability will be announced through the AVP Newsletter. To receive continuing education credit after May 31, 2026, participants will need to take the assessment when the continuing education module is reactivated.

Please note that the shelf life of the COVID-19 vaccine that is being sent to providers is shorter than it was at the start of the season and will continue to decrease as we near the end of the season.
To effectively manage this reduction in shelf life, the CDC recommends that providers order smaller quantities of the vaccine - enough for 3 to 4 weeks of usage - and place more frequent orders if necessary. Before placing an order, always check the Vaccine Advertisements for an available transfer in your area.
