We Want Your Feedback! Complete the Provider Survey

The Department of Health is collecting valuable feedback about your satisfaction with the Childhood Vaccine Program (CVP) operations and service delivery. This survey will take about 2 minutes to complete, and the information will be used by the Department of Health to improve customer service and provider support.

CVP Training on Thursday

DOH Sends Reminder Letters to Families

This week, the Department of Health started sending letters to parents of children ages 4-6 years old whose Washington Immunization Information System record indicates they are missing vaccines needed to start school. The letter asks the parent to follow up with their child’s health care provider for questions and to receive additional vaccinations. There is additional information for parents about recommendations for older children related to HPV vaccination at 9, meningococcal vaccine and how to access help to schedule an appointment.
