This email is being sent to COVID-19 Vaccine Providers, COVID-19 Vaccine Local Heath Jurisdiction Partners, and Emergency Response contacts.
Dear COVID-19 Vaccine Provider,
Enrolled COVID-19 Vaccine Program providers will need to start submitting temperature logs for each storage unit currently listed on the provider agreement April 1, 2022, this means starting March 1, 2022 your facility location will need to ensure temperature logs are being recorded by an approved method.

Upcoming Temperature Monitoring Requirements for COVID Vaccine Program Providers:
- Starting April 1, 2022, temperature log submission are required for each storage unit currently listed your provider agreement.
- This means starting March 1, 2022, your facility location will need to ensure temperature logs are being recorded by an approved method.
- Temperature logs will be collected using the REDCap system. An email will be sent out to primary and backup vaccine coordinators at the beginning of the month notifying providers that temperature logs are due and will include a link to upload the temperature log(s) for the month.
- Temperature logs will need to be submitted for each storage unit listed on the COVID Vaccine Program provider agreement.
Acceptable temperature logs submission forms:

Childhood Vaccine Program (CVP) Temperature logs: Providers enrolled in CVP may submit the temperature logs approved for use in the CVP program. All CVP providers are required to continue to follow ALL requirements of that program, including process for submitting required temp logs to CVP.
- Digital Data Loggers (DDL)/Temperature Monitoring System (TSM) Summary Reports:
- These are acceptable with prior review/approval by the program
- You will need to email a copy of the DDL summary report to covid.vaccine@doh.wa.gov for prior approval.
- The summary report will need to display the following:
- One month of temperature data
- Data points listed on the CDC paper temperature logs. For example, daily min/max record, graph displaying month data trend, and clearly illustrates whether your unit experienced an excursion.
Monitoring vaccine storage equipment and temperatures are daily responsibilities to ensure the viability of your vaccine supply and the safety of your patients. Implementing routine monitoring activities can help you identify temperature excursions quickly and take immediate action to correct them, preventing loss of vaccines and the potential need for revaccination of patients.
Current temperature monitoring requirements include checking and recording storage unit’s minimum and maximum temperatures at the start of each workday. If your Digital Data Logger (DDL) does not display min/max temperatures, check and record the current temperature a minimum of 2 times per workday (at the start and end of the workday). You also should be reviewing DDL temperature data daily to check for temperature excursions. Download and review DDL temperature data weekly for changes in temperature trends that might require action. This will help identify any storage unit malfunctions and will reduce the risk exceeding the data capacity of the DDL data storage.
