Community Feedback: Equitable Planning for Vaccine Boosters

  • The WA State Department of Health is gathering feedback on booster vaccine planning to gather insight, feedback and perspective to ensure equitable and accessible implementation strategies and outreach. Please take a few minutes to share your thoughts via Feedback Survey by October 15th, 2021, end of day.  
  • If you need this survey in an alternative format, please contact Fathiya Abdi, Fathiya.Abdi@doh.wa.gov.  We can provide the questions in plain text form, in a Word document, or conduct the survey with you by phone
