Customer Service Counter Closure
Effective immediately, the Customer Service Counter at the Tumwater, Town Center 2 site, will be closed to decrease the risk of spreading the Coronavirus.
Application Information
Washington State is currently under a governor proclaimed state of emergency, resulting in an increase to nurse licensure application volume. To provide the best customer service, please email all questions to nursing@doh.wa.gov or leave a detailed voice message and a customer service representative will respond in one business day.
A customer service representative will email you with detailed instructions if anything is missing from your application. If you want to check the status of your pending application, please check the Department of Health’s Provider Credential Search.
Online Applications
Please review the Online Application Instructions for directions on how to apply for a nursing credential.
If you meet the criteria below then you will need to request a paper application by emailing nursing@doh.wa.gov:
- You have previously applied for a license in Washington state and your application was closed as deficient. You can check for a closed application on the Department of Health’s Provider Credential Search.
- You don’t have sufficient information in the public record to verify your identity.
- You are applying for reactivation of a previous license.
You will need mail your completed paper application and fee to:
