Athletic Physicals for FCPS HS Student-Athletes Offered Locally
VHSL Athletic Paperwork
While we are waiting on a decision from the VHSL on how athletics and activities will proceed for the 2020-2021 school year, this is a good time to get your VHSL Physical Form completed. You can make an appointment with your personal doctor or take advantage of the free or low cost sports physicals for the upcoming school year.
You will need to complete all paperwork and turned into Activities Office and coaches in order to participate in any green day work out and to try out.
We will send more information on when we will begin to collect them shortly. Link is below for all requirements.

Are you interested in playing a sport at South Lakes?
Below are four items all student/athletes must have to try-out. Please check each individual sport page to see what else may be needed such as a try-out criteria or weight room form. All forms can be found on our website, https://southlakesathletics.net/
- VHSL Physical form to be filled out by a doctor. Must be dated AFTER May 1, 2020. Turn into the Activities Office.
- Concussion Education for athletes and parents: PDF reviewed by both the athlete and parent. Turn in the last page, signed, to the Activities Office with your VHSL Physical form. This must be completed each year.
- Emergency Care Card. Print & Complete. Turn into your coach. This must be done each season.
- Must also complete the Student/Athlete Survey HERE.
