The required forms listed below are due the first day of school to student's third period teacher.

Emergency Care Information

All students must have accurate emergency care information.  Parents can update this information through WeCare@School. Update no later than Monday, August 26th.  A completed signed emergency care form should also be returned on the first day of school to their third period teacher.

Student's Rights and Responsibilities

Review the Student's Rights and Responsibilities with your child.

Bring the signed  Parent and/or Guardian Signature Sheet on the first day of school to their third period teacher.

Health Information Form

Complete the Health Information Form and turn in the first day of school to their third period teacher.

AP Exam Fee Structure (New!)

Starting with the 2019-2020 school year, the College Board is launching MyAp, a digital platform which includes new AP test ordering timeline, fee structure and instructional resources.  As a result, FCPS had to adjust the AP Test Opt-Out timeline and fees.  AP students must now make Opt-out decisions for AP tests in October 2019 and parents will be billed in December 2019 for tests that exceed the six FCPS funded tests.  Per College Board’s new fee structure, AP students that do not sit for a registered AP test after November 15th 2019 for reasons that include forgetting to opt-out, dropping an AP course, not sitting for the test on exam day etc., will be assessed a$40.00 change fee.  Please be sure to review the materials on the LHS website.
