Dear Parent,
Fairfax County Public Schools (FCPS) is studying how school environments relate to discipline practices in the Division. As part of the study, FCPS is asking parents to share their views of their child’s school environment and how discipline is approached in their child’s school.
It should take approximately 15 minutes for you to complete the survey. If you have more than one child in an FCPS school, please complete the survey thinking about the school that your oldest child attends. If you wish to complete the survey again for schools your other children attend, you are welcome to do so.
Your responses will be anonymous and will never be personally identified with you, your child, the school’s principal, or your child’s teachers. Your participation is not required, but is very important to help FCPS understand school environments and discipline and make improvements. The survey is available until February 22, 2019.

A student survey will also be administered in a small number of FCPS classrooms. Your child may be asked to participate in this survey about their perceptions of the school environment and discipline. This will be administered at school and, again, results will be anonymous. Participation is voluntary for your child.

The Fairfax County Health Department (FCHD) employs school health aides and public health nurses who provide school health services to the students in Fairfax County Public Schools (FCPS). To determine the quality of these services, a survey has been developed by the FCHD to be shared with all parents or guardians and school staff members who interact with the school health rooms. This may include principals, school counselors, and main office staff members. The survey has been approved for distribution by the FCPS’ Internal Survey Advisory Committee.
