    Lost and Found

Now that spring/summer weather has finally arrived please help us clear out the Lost and Found.  Please come in and check out the Lost and Found before May 25.

After Memorial Day, we will bag up what is remaining and distribute to various charities.

    School Supplies

Fliers came home in Friday Folders for ordering supply kits for the 2018-19 school year. Parents can order "online only" this year from Campus Survival Kits.

To order please go to:    http://www.campussurvivalkits.com/poplartree

    Parent Input

As we look ahead to the 2018-19 school year, we are asking for your input. We value our partnership with home and school and appreciate your thoughts on your child's learning needs.

Please fill out this form by June 15 in order to provide some information about the best learning environment for your child. We will use this information as one piece for classroom placements.

    Immunizations

Important Immunization Requirement: Do You Have a Student Entering Sixth Grade Next Year?

If yes, he or she may need a booster dose of Tdap (tetanus, diphtheria, acellular pertussis). Sixth grade students cannot begin school in August without documentation of the Tdap booster. Parents or guardians should ask their private healthcare provider, or the health department, to review their child’s immunization record if they are unsure if their child has received the Tdap booster. Students who have received a Tdap booster between the ages of seven and eleven do not need another dose. Documentation must be provided to the school student information assistant (SIA) or registrar as soon as possible. Learn more about the Tdap booster
