Rising 6th Grade TDAP (Tetanus, Diphtheria, Acellular Pertussis) Immunization due prior to entering sixth grade.

Please
check your child’s immunization records carefully. Your child is required to
have a Tdap booster.
·
If
they have not received a Tdap booster dose after age seven (7) years, we ask
that your child receive this vaccine and that you provide documentation to the
school prior to June 16, 2017.

Documentation
can be a copy of your child’s immunization record, or a note on letterhead from
your healthcare provider listing the date, month and year of his or her most
recent Tdap booster.
If
you have medical questions about the immunization, please contact the school
public health nurse at 703-239-4000. For
questions regarding immunization documentation, please contact the school
student information assistant (SIA) or registrar.
