Revised Initial and Renewal Applications

The Office of Child Care Health and Safety (OCCHS) is pleased to announce that the initial and renewal applications for licensure and voluntary registration have been revised. The goal was to streamline information collected, condense the number of pages, remove additional requirements to submit certain items at renewal unless a change has occurred, and to clarify language within the application.
The revised applications can be found online on the Department’s Child Care VA website. Simply locate your program type on the Program Type webpage and select the appropriate link.
In the event that you have already submitted an application or are in the process of completing a previous version of an application, the OCCHS will continue to accept a previous version for 30 days. However, all programs are encouraged to begin using the revised applications immediately to reduce the amount of documentation required for your application.
Programs are reminded that § 59.1-69 of the Code of Virginia requires that any person (i.e. entity), including any individual, corporation, partnership, association, cooperative, limited liability company, trust, joint venture, government, political subdivision, or any other legal or commercial entity, file for a fictitious name through the Virginia State Corporation Commission (VSCC) prior to doing business under a name different from that of the legally operating entity. The Department requires proof of a fictitious name through the VSCC prior to including it on a license or certificate of registration. Additional information about fictitious name requirements and how to file for a fictitious name can be found on the VSCC website.
Programs are also reminded that initial and renewal applications should be submitted through the online provider portal. Utilizing the portal will help avoid mailing delays and ensure that your documents are submitted securely to the OCCHS. A quick reference guide on using the portal is available on the Department’s Child Care VA website. As a reminder, the Department is not collecting licensing application fees through June 2025 and no longer collects application fees for voluntary registration.
