Important Update – OHB Fax Machine
Due to a technical issue, the OHB fax machine is currently out of service. We are working to restore service as quickly as possible; however, an estimated restoration time is not yet available.
Other options available to you include sending forms via email (ohb@dhrm.virginia.gov) or via the United States Postal Service.
Any forms that were faxed after 5:00 p.m. on Friday, April 3, 2026, should be resent. We apologize for any inconvenience this has caused.
