Introducing Community Connected Care: Bridging the Gap between Social Needs and Healthcare Access
COVA Care and COVA HDHP members can now access Community Connected Care (CCC), a program designed to help members facing social challenges by linking them to vital community resources. Community Connected Care aims to ensure that every member can fully access the healthcare system and receive the necessary care, regardless of their social circumstances.
Anthem’s Community Connected Care program can assist with locating community-based resources to help members with family matters and other physical and mental health concerns that could impact their health.
GroundGame.Health is Anthem’s Community Connected Care partner for this program. A Care Coordinator from GroundGame.Health may reach out to assess needs then serve as an advocate, connecting them to health benefits, local services, and organizations in the community that can provide the most effective support. Participation in the program is completely voluntary and confidential.
Please share the flyer and FAQ with employees to learn more about Community Connected Care so that employees and their families can benefit from these services. If an employee would benefit from this free program, please have them call Anthem at 1-800-552-2682 or visit Anthem |GroundGame.Health.
In addition, the Department of Human Resources Management, Office of Health Benefits will be sending members an introductory program email in the coming weeks.
Emergency Room (ER) Alternate Solutions
COVA Care and COVA HDHP members will begin receiving emails explaining appropriate ER and urgent care usage. Knowing the best place of service for treatment not only saves the member time and money, but it also lowers the cost of claims for the health plan.
Additionally, members who have visited the ER for non-emergent reasons may receive targeted emails throughout the year.
No Annual PCP Preventive Exam
COVA Care and COVA HDHP members who have not had an annual preventive exam within the last 12 months may receive an email encouraging them to contact their PCP to get a preventive exam. When an in-network provider is used, the health plan provides 100% coverage for preventive care exams.
The health plan offers a variety of options for preventive care visits such as a traditional PCP visit, virtual primary care through LiveHealth Online and a virtual checkup home kit through Catapult Health. Catapult Health may also be sending direct emails to members throughout the year about the convenient, simple and comprehensive health checkup that can be done right at home.
Please do not reply to this e-mail. You may send inquiries to the Office of Health Benefits mailbox at ohb@dhrm.virginia.gov.
