SUBJECT: Mandatory Report Request: EEO Complaint and Grievance Resolution Report
Dear Human Resource Directors,
DHRM is introducing a new, semi-annual mandatory report: the EEO Complaint & Grievance Resolution Report. The primary goal of this report is to gather high-level data from agencies on the outcomes of internal investigations concerning employee relations issues, specifically related to EEO discrimination complaints and grievances. On numerous occasion, we receive inquiries regarding the total number of EEO complaints and/or grievances issued statewide, and we do not currently have access to this information. Having total numbers by agency will help us address these types of questions.
The report template includes two key tabs: “EEO Complaint Report” and “Grievance Resolution Report”. You are required to complete both tabs, and detailed instructions for each can be found directly under the respective tabs. This report will be due by January 8, 2025. Please submit the report using this form.

We will be working to add this report to the mandatory report list and send more information regarding the next due date in six months. Thank you for your cooperation!
