You’re eligible for this benefit as a member of COVA Care or COVA HDHP. Once qualified under the 1-minute survey, program participants will receive instant, unlimited digital coaching and daily education to prevent the chronic condition of diabetes. Goals are directly tied to weight management through healthy eating, activity, stress management and improved sleep. There is no cost to participate in this program. Participants will also receive a smart scale upon enrollment and the chance to earn a Fitbit.

Lark's fully CDC recognized Diabetes Prevention Program (DPP) is a personalized digital
health solution that uses A.I. coaching and connected devices to help your members lower
their risk for type 2 diabetes. The smartphone-based platform provides 24/7 care coaching
that helps members take small steps over time to create lasting behavior change and improve health outcomes. Eligibility requirements for the Lark Diabetes Prevention Program include qualifying as prediabetic according to a survey designed by the Centers for Disease Control and membership in a participating health plan. You may be eligible to earn health-related devices such as a scale at no cost to you. The ability to earn health-related devices may vary by health plan and may contain minimum program engagement requirements, such as weighing in, completing missions with your digital coach, and logging activity or meals. Eligibility determinations are made by Lark at its sole discretion.
