IMPORTANT REMINDERS

ENROLLMENTS AND CHANGES

As a reminder, please submit all TLC forms (enrollment, group adjustment and personal data change) via email to TLC@dhrm.virginia.gov, via fax to 804-371-0231 or by US mail to The Local Choice (address noted on form).

If you send forms via fax, please use the alternate, temporary fax number 804-371-0231 as noted on the September 2, 2021 E-News.

We are currently processing October 1 enrollment requests so your Anthem bill, due October 1, may not reflect all of your October 1 transactions.

It is very important that you review your HuRMan reports to confirm all forms are processed correctly.

• The daily BES Turnaround Document is the official record of changes made in BES before 5:30 p.m.
• The BES Enrollment Report is available on the 3rd, 10th, 17th and 24th of each month. It lists eligible participants as of the first of a month. The report available on September 24, 2021  reflects the total group enrollment effective October 1, 2021.

Anthem’s Employer Access is the easiest way to get fast access to the latest tools to help you better manage your plan. The tool allows you to view and download your bill as soon as it is available, make payments online, and request member ID cards.

TLC Inquiries

Please direct ALL TLC inquiries to the email address (TLC@dhrm.virginia.gov). Inquiries are generally handled within 2 – 3 business days. Those requiring research or investigation make take 5 – 7 business days.
