VIONE, or Vital, Important, Optional, Not needed, Every medication has an indication, is a methodology to identify and discontinue unnecessary medications that can harm or burden Veterans who are taking multiple medications. VIONE recently reached the milestone of impacting over 1 million Veterans, successfully stopping more than 2.6 million prescription orders with the potential to harm.

IMPROVING COMMUNICATION IN HOME-BASED PRIMARY CARE AT AAHCM MEETING

On October 19, Rebecca Haynes and Kelli Jones from Improving Communication in Home-Based Primary Care presented at the American Academy of Home Care Medicine 2024 Annual Meeting in Phoenix, AZ. The team shared how to facilitate effective two-way communication and modify templates to fit the needs of any home-care organization.

PACT FUNCTIONAL STATUS SCREENING INITIATIVE

The PACT Functional Status Screening Initiative is a streamlined approach to help PACTs identify and manage functional impairment for older Veterans. In pilot testing, the intervention led to screening of 100% of eligible Veterans and high rates of satisfaction among patients and clinicians, with lower perceived burden. Participating sites have their existing Katz/Lawton functional status screener replaced with an updated, tailored tool to identify referrals and services to help improve care and outcomes for Veterans.
