	The Pap Hub and Spoke Program is an evidence-based care delivery model that promotes screening and vaccination to reduce the risk of women Veterans dying from cervical cancer. Launched in January 2022, the program has improved cervical cancer screening participation, timely results communication, and the management of abnormal test results, as well as received excellent provider and patient satisfaction ratings. The program won the VA Women’s Health Research Impact Award and Pitch of the Day as a 2023 VHA Shark Tank Competition finalist. Cohort 9 Promising Practices

Innovators of Diffusion’s 9^th cohort of Promising Practices, winners of this year’s VHA Shark Tank Competition, will attend Diffusion’s Base Camp in January 2024 to develop a plan for replicating their practices to new VA facilities. The practices each align with one of the VA health care priorities: (1) accelerate VA’s journey to a High Reliability Organization, (2) prevent Veteran suicide, (3) support Veterans’ whole health, caregivers and survivors, and (4) support Veterans’ whole health, caregivers and survivors.

Surviving SEPsis90 Campaign – HRO Saves Lives! provides a framework to improve outcomes for Veterans with sepsis.
RAVEE: Reaching All Veterans at Every Encounter utilizes an interdisciplinary approach to tackle Veteran suicide.
Virtual Waiting Room addresses a lack of formalized check-out processes for telehealth.
Therapeutic Carbohydrate Reduction (TCR) Aspire uses whole health coaching to support Veterans with type 2 diabetes and other metabolic health conditions.
SHRIMPS Surgical Intraoperative Handoff Initiative standardizes critical information to be conveyed during surgical technician staff changes in the operating room.
Veterans Impact Project (VIP) enables social worker staff to address Veterans’ emergent and unique needs immediately, even in the absence of community resources.
SCOUTS (Supporting Community, Outpatient, Urgent Care and Telehealth Services) is a post emergency department complementary service that supports care transitions.
Suicide Risk Screen Follow Up Monitor provides a CPRS pop-up notification and populates a real-time report to identify patients that are deemed at-risk.
From Resistance to Resilience: A Veteran’s Renewal with Ketamine is a stand-alone clinic that offers Ketamine therapy for Treatment-Resistant Depression to Veterans.
One View Unified Dashboard provides a unified dashboard that consolidates view alerts from multiple facilities onto a single display.

The Office of Geriatrics and Extended Care is leading a second VA Age-Friendly Action Community from December 2023 to July 2024. iNET Incubator Series Final Session

VHA Innovators Network (iNET) is wrapping up this year’s edition of the Incubator series, a six-part virtual workshop experience open to all VA employees. The Incubator series will host its final session on Thursday, December 14. Highlighting The Surgical Pause
The National Quality Forum Webinar recently featured the Surgical Pause, a National Diffusion Practice. Dr. Daniel Hall shared findings from the Surgical Pause intervention that can identify frail patients in as little as 30 seconds to support goal clarification prior to surgery, empower some patients to elect nonoperative treatment plans, and offer strategies to mitigate frailty associated risks.
