We are receiving numerous secure messages regarding VA patient COVID-19 vaccine status in the Connecticut state vaccine portal (CT WIZ). Click the picture below to learn more about CT WIZ
VA is currently not sharing vaccination data with the state. If you wish to participate in this state program, use this link to upload your photo ID and your vaccination card: https://dphsubmissions.ct.gov/ImmunizationUpdate

WANT TO GET YOUR IMMUNIZATION REPORT THOUGH MHV?
If you would like to get your immunization report through the VA Patient portal, My HealtheVet (MHV) follow the directions below.
TO DOWNLOAD YOUR VA IMMUNIZATION REPORT
- Click “Blue Button Medical Reports” under HEALTH RECORDS box/tile/section
- Click the entire blue tile/box that says VA BLUE BUTTON REPORT
- Select a date range & select the types information you would like VA IMMUNIZATIONS
- Click the “Submit” button (you may have to click a CHECK UPDATE button)
- To view, select either “Download PDF or TEXT File” or “View/Print

If you are unsure if you have a My HealtheVet account, you can call 877-327-0022 from 8a-8p M-F to check.
- If you have not set up a My HealtheVet account yet, you can do so from the comfort of your own home by registering at www.myhealth.va.gov
If you have an account but have a letter (B) or (A) next to your name when you log in. you NEED the free upgrade to Premium (P).
