What
1st, 2nd and 3rd doses of Pfizer vaccine for employees, Veterans, spouses, caregivers and CHAMPVA recipients under the authority of the SAVE LIVES Act (SLA). Last Pfizer dose must have been administered at least 6 months ago.
Notes
Veterans, spouses and SLA need to have eligibility established.
If you’ve had a previous COVID-19 vaccine dose, please bring your CDC card with you when presenting for your vaccination.
