Covid-19 Vaccine Update
Veterans Health Administration sent this bulletin at 06/04/2021 01:56 PM EDTDear Veteran:
The Columbus VA is working hard to ensure you are vaccinated against Covid-19.
If you have not received the Covid-19 vaccine, you can walk-in to the Chalmers P. Wylie VA Ambulatory Care Clinic, Monday thru Friday from 8:00 a.m. to 4:00 p.m. to get your vaccine. We administer the one-dose Johnson & Johnson or two-dose Moderna vaccines.
The Johnson & Johnson vaccine is also available at our Community Based Outpatient Clinics in Grove City, Marion, Newark and Zanesville by appointment only, Monday thru Friday, 9:00 a.m. to 3:00 p.m. You can schedule your appointment at one of our CBOCs by calling:
- Grove City: 614-257-5804
- Marion: 614-257-5200
- Newark: 614-257-5200, ext. 2970
- Zanesville: 740-453-7725
If you received the vaccine through another healthcare provider or at a location other than the Columbus VA, please call your primary care nurse to let them know. This will help us keep your medical records current.
When you call in, have your Covid-19 vaccine card nearby so you can provide the following information:
- Your Name
- Last 4 of SSN
- Lot# of vaccine (if you received the Pfizer or Moderna Vaccines, provide Lot# from both doses)
- Location you received vaccine
- Date for each dose of vaccine
The Columbus VA now is vaccinating all Veterans, Veteran spouses, and Veteran caregivers. This includes ALL prior service personnel - Active Duty, Reserve, or National Guard – regardless of time in service, duty status or character of discharge.
If you are not enrolled for healthcare with the VA, please stop by the Enrollment Office on the 1st Floor of the Chalmers P. Wylie VA Ambulatory Care Center from 8:00 a.m. to 4:00 p.m. to register before you get your shot. For more information on registration call the Columbus VA Enrollment Office at 614-257-5608.
Veterans coming in to receive the vaccine will receive a $10 VA Canteen Services voucher for use at our VA Canteen facilities.
