Announcement: Fingerprint Card Instructions for Use in Obtaining Suitability Clearance to Access the New Ticket Portal

The Social Security Administration has mailed the FD-258 FBI Fingerprint Cards to the State Vocational Rehabilitation (VR) agency point of contacts (Signatory Authorities). Attached are the instructions for the applicants filling out these cards. Please submit two complete fingerprint cards per applicant.  If your office has more than two individuals that need Suitability clearance, please contact Natalie Sendldorfer at (410) 966-5696 to request more fingerprint cards.

Thank you for the prompt responses with your completed applicant lists. If you have not yet returned your completed applicant list, please fax it to the Office of Suitability at 410-966-0640 with a courtesy fax to Natalie Sendldorfer at 410-966-3280 by September 29, 2014. Please write your VR state code on the line next to “Contract Number.”
