Announcement: Updated Unassignment Form Now Available

On Thursday, August 17, 2017, Social Security implemented a new requirement for Ticketholders who request to unassign their Tickets. Ticketholders are now asked to provide the reason they are unassigning their Ticket. The unassignment form on the Choose Work website has been updated to reflect the new directive and can be downloaded now.

As a service provider, if you assist clients with unassigning their Tickets, make sure that you use the current form and have your clients submit complete requests.

The unassignment request includes:

  • Ticketholder’s name
  • Ticketholder’s phone number
  • Ticketholder’s SSN
  • Name of the provider being unassigned
  • Reason for the unassignment
  • Ticketholder or Representative Payee’s signature
  • Date
