Call-in Number: 800-619-7898/ Participant Code: 4096663

The Fiscal Year (FY) 2012 Supplemental Funding for Quality Improvement in Health Centers is a one-time supplemental funding opportunity under Section 330(k) of the Public Health Service Act to improve access to services, quality of care, and clinical outcomes through the patient centered medical home (PCMH) model of care. The supplemental funding focused on improving outcomes related to cervical cancer screening for health center patients by supporting PCMH transformation projects. Health centers that received funding under the supplemental are required to submit interim and final reports. The Interim Report for the FY 2012 Quality Improvement Supplemental will be released in HRSA’s Electronic Handbook (EHB) by April 29, 2013 and must be submitted by June 3, 2013.

In an effort to support this reporting requirement, HRSA will be hosting a technical assistance session to review the interim report requirements and submission process.   Please find the information for the session below.

Presentation materials will be available the week of the session at: http://www.hrsa.gov/grants/apply/assistance/pcmh/. To join the meeting, use the following link: https://hrsa.connectsolutions.com/fy12qigrantee/.
