Workforce Issues

Competency Exam for Health IT Professionals

Through an arrangement with the Office of the National Coordinator for Health Information Technology (ONC ), HRSA has secured a limited number of vouchers for the health IT professional competency exams. These exams enable health IT professionals, employers, and other stakeholders to assess and validate their health IT competency levels. The exams may also be used by employers to identify training gaps and personnel needs integral to achieving meaningful use of electronic health information.

This voucher waives the exam fee for one time only and provides the opportunity for health IT professionals to take one of the six exams offered. Individuals and health IT professionals interested in working with safety net providers can request a voucher for taking this professional competency exam.

National Health Service Corps (NHSC) Loan Repayment Program (LRP)

The 2013 NHSC Loan Repayment Program (LRP) application cycle is now open through April 16, 2013. The program offers primary care medical, dental, and mental and behavioral health providers the opportunity to have their student loans repaid while serving in communities with limited access to care. Affordable Care Act Helps Expand the Use of Health Information Technology

New resources announced to support 37 Health Center Networks across the United States.

HRSA announced new grants that will improve the quality of care at community health centers through implementation and adoption of health information technology (HIT). The grants, totaling more than $18 million in Affordable Care Act funds, will support 37 health center networks made up of health centers across the country.

Health Center Network grants will support the adoption and meaningful use of certified electronic health record technology and technology-enabled quality improvement strategies used in health centers. The networks, comprising at least 10 collaborating health center organizations, are designed to promote enhanced sharing of their respective knowledge and expertise. This network sharing model will also address key operational and clinical needs through a greater program integration.

Oral Health Quality Improvement Module

The oral health quality improvement module is now available on HRSA’s website.
HRSA recently developed an Oral Health Quality Improvement module, through a contract with the American Academy of Pediatrics, to educate primary care providers working with children on the uptake of oral health prevention activities into their systems of care. Since the incorporation of oral health activities into a medical practice involves systems change, quality improvement approaches can be more effective than simple didactic training. This module is directed towards training primary care providers serving vulnerable and underserved children to improve oral health and oral health access of this population. The Oral Health Quality Improvement module is available to the public at no charge.

New Rule Protects Patient Privacy, Secures Health Information

The U.S. Department of Health and Human Services (HHS) strengthens privacy and security protections for health information established under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

The final omnibus rule greatly enhances a patient’s privacy protections, provides individuals new rights to their health information, and strengthens the government’s ability to enforce the law. The changes announced expand many of the requirements to business associates of these entities that receive protected health information, such as contractors and subcontractors. Penalties are increased for noncompliance based on the level of negligence with a maximum penalty of $1.5 million per violation. The changes also strengthen the Health Information Technology for Economic and Clinical Health (HITECH) Breach Notification requirements by clarifying when breaches of unsecured health information must be reported to HHS.

New Healthcare Information and Management Systems Society (HIMSS) Resources: HIPAA and Stark and Anti-Kickback 

HRSA’s New Clinical Quality and Performance Measures Toolkit

HRSA’s Clinical Quality and Performance Measures Toolkit now available.

The HRSA Clinical Quality and Performance Measures Toolkit was developed to specifically address the needs of safety net providers seeking to initiate or upgrade their quality activities. This toolkit features proven strategies and techniques to implement or enhance a safety net provider organization's new or existing quality improvement program. The toolkit's wide range of practical, convenient, and useful tools support data collection, program implementation, and performance measurement.

The toolkit also contains clinical quality measure modules focused on critical primary care and chronic disease topics including, but not limited to: screening for cervical, colorectal, and breast cancer; diagnosis and management of diabetes; and screening and hypertension control. These quality improvement resources are adaptable and have been compiled from successful quality improvement initiatives.

Rural Health Network Development Planning Grant (Network Planning) Program

Program overview and eligibility.

The Rural Health Network Development Planning Grant (Network Planning) program is authorized by the Public Health Service Act, Section 330A (f) (42 U.S.C. 254(c)(f)), as amended. The legislative purpose of this program is to expand access to, coordinate and improve the quality of essential health care services and enhance the delivery of health care in rural areas. This program provides one-year grants up to $85,000 and allows applicants to develop a business or strategic plan, conduct a needs assessment, conduct health information technology readiness and ultimately form a network. Funds cannot be used for direct delivery of health care services.  Successful grantees often apply for the 3 year Network Development implementation grant to continue the work they started under the Network Planning grant.

Lead applicants must be:

  • Located in a Rural County or Eligible Rural Census Tract within an urban county;
  •  Public and non-profit entities including faith-based and community organizations; and
  •  In a consortium with at least two additional organizations. These two other organizations can be rural, urban, non-profit or for-profit. The consortium should include three or more health care providers that provide or support the delivery of health care.

In recognition of American Heart Month, February’s health IT grantee spotlight features Marshfield Clinic Research Foundation in its development of the Heart Health Mobile app to identify and reduce cardiovascular disease risk.

Quality improvement grantee spotlight
February’s quality improvement grantee spotlight focuses on Project Renewal and its innovative approaches to provide high quality patient care to homeless individuals in New York City. This spotlight highlights how Project Renewal has aligned with national quality initiatives to improve the screening and control of hypertension within its patient population.

Agency for Healthcare Research and Quality (AHRQ) Updates

  • Clinical quality measures useful in estimating quality across different EHRs -- short video  available or read the abstract through PubMed.
  • New podcast for patients with diabetes with low health literacy highlights work to address low health literacy among patients with diabetes and features Margaret Handley, MPH, PhD, of the University of California, San Francisco Center for Vulnerable Populations.
  • TeamSTEPPS® training module for primary care teams now available for primary care practices. Primary care practices can use this training curriculum to improve patient safety by teaching health care providers and staff how to communicate better, work more effectively, and make a greater commitment to teamwork.
  • AHRQ study examines use of clinical decision support systems to promote judicious use of antibiotics. A recent AHRQ-funded study examined facilitators and barriers to the use of a clinical decision support system (CDSS) that targeted inappropriate antibiotic prescribing. The study, “Adoption of a Clinical Decision Support System to Promote Judicious Use of Antibiotics for Acute Respiratory Infections,” appeared in the August 2012 issue of the International Journal of Medical Informatics.

Meaningful Use Updates

  • More than 190,000 providers received incentive payments. As of December 2012, more than 190,000 health care providers had received incentive payments for participating in the Medicare and Medicaid Electronic Health Record (EHR) Incentive Programs.
  • More than $10.4 billion in incentive payments made. As of December 31, 2012, more than $6.1 billion in Medicare EHR Incentive Program payments has been made (since May 2011) and more than $4.3 billion in Medicaid EHR Incentive Program payments has been made (since January 2011, when the first set of states launched their programs).

  • Mobile Devices: Know the RISKS. Take the STEPS. PROTECT and SECURE Health Information, an educational initiative launched by the U.S. Department of Health and Human Services (HHS), includes a set of online tools and practical tips on ways to protect patient health information when using mobile devices. Educational resources include videos, easy-to-download fact sheets, and posters to promote ways to safeguard patient health information.

National Quality Forum (NQF) Updates

NQF appoints new president/CEO exit disclaimer. The NQF announced the appointment of Dr. Christine Cassel as the organization’s next President and CEO, effective mid-summer 2013.

 International Classification of Diseases, 10th Edition (ICD-10) Update

 

  • Final rule delays compliance date. On August 24, 2012, HHS Secretary Sebelius announced a final rule  that delays the compliance date for use of the new International Classification of Diseases, 10th Edition diagnosis and procedure codes, or ICD-10, until October 1, 2014. ICD-10 includes codes for new procedures and diagnoses that provide enhanced information for quality improvement and payment purposes.

*Colleges Receive Additional Funding for Health IT Programs*

In late September, the Department of Labor (DOL) [ http://www.dol.gov/ ] announced $500 million in grants [
http://www.doleta.gov/taaccct/ ] to community colleges and universities across the nation to develop and expand
workforce programs. Among the DOL grant recipients, three community colleges plan to use the funds for health IT-related
programs. These programs include:


* Bellevue Community College in Washington State [ http://depts.bellevuecollege.edu/informatics/ ] exit disclaimer [
http://www.hrsa.gov/exitdisclaimer/hrsaexitdisclaimer.html ], which will receive about $11.8 million to provide health
IT career training to veterans and their eligible spouses;

health informatics specialist certificate program; and

Office of the National Coordinator  for Health Information Technology (ONC) Privacy and Security Staff Training Game Now
Available

In September, staff from ONC and the Health Resources and Services Administration (HRSA) produced a webinar on ONC’s new
Privacy and Security training game and how this resource can be used by safety net providers to better understand how to
comply with federal privacy and security guidelines. The game can be used to accredit staff for HIPPA privacy training,
as well as teach all staff within a provider environment the importance of privacy and security issues concerning health
information technology (IT).

Affordable Care  Act Funds to Enhance Quality of Care at Health Centers

Health and Human Services (HHS) Secretary Kathleen Sebelius announced new grants [
http://www.hhs.gov/news/press/2012pres/09/20120927b.html ] that will improve the quality of care at community health
centers and ensure more women are screened for cervical cancer. The grants will help 810 community health centers become
patient-centered medical homes (PCMHs) and increase their rates of cervical cancer screening.

“Our health centers are committed to providing high quality health care services and today’s awards help continue these
efforts,” said Secretary Sebelius.

The patient-centered medical home is a care delivery model designed to improve quality of care through better
coordination, treating the many needs of the patient at once, increasing access, and empowering the patient to be a
partner in their own care.

These awards will provide assistance to 810 health centers as they make the practice changes, such as improved care
coordination and management, that are necessary to become patient-centered medical homes. The awards will also support
health centers’ efforts to increase the percentage of women screened for cervical cancer.

Health Information Technology Grantee Spotlight "Colorado’s Community Managed Care Network: Hiring Veterans to Help
Health Centers with Health IT"

In recognition of the White House’s Veterans Hiring Initiative and Veterans’ Day, the Health Resources and Services
Administration is pleased to highlight the Colorado Community Managed Care Network (CCMCN), a network of health centers,
which successfully achieved meaningful use thanks to the leadership of an employee who is also a Veteran. Mr. Francisco
Perez joined CCMCN a year ago, bringing with him project and logistical management experience as well as a strong
background in teamwork and mission-oriented customer service that have proven to be a tremendous asset to CCMCN.

This month, in recognizing Veterans’ Day, the Health Resources and Services Administration’s (HRSA) Quality Improvement
website highlights the work of the Fort Drum Regional Health Planning Organization (FDRHPO), a non-profit health network
in Northern New York State. Fort Drum is one of the nation’s largest military installations and home to nearly 19,605
active duty military and 19,820 military family members Fort Drum and FDRHPO work together to enhance access to
healthcare services for Fort Drum soldiers and families, and the surrounding veteran and civilian community.

The HRSA Clinical Quality and Performance Measures Toolkit [ http://www.hrsa.gov/quality/toolbox/ ] was developed to
specifically address the needs of safety net providers seeking to initiate or upgrade their quality activities. The
toolkit features proven strategies and techniques to implement or enhance a safety net provider organization's new or
existing quality improvement program. The toolkit's wide range of practical, convenient, and useful tools support data
collection, program implementation, and performance measurement.

The toolkit also contains clinical quality measure modules focused on critical primary care and chronic disease topics
including, but not limited to: screening for cervical, colorectal, and breast cancer; diagnosis and management of
diabetes; and screening and hypertension control. These quality improvement resources are adaptable and have been
compiled from past and current successful quality improvement initiatives.

National Quality Forum (NQF) Endorses Phase II Population Health Measures

The NQF Board of Directors has endorsed five healthy lifestyle behaviors and broader population health measures, focused
on community interventions that can improve health and well-being.

NQF has previously endorsed population-level measures, but this is the first endorsement project primarily focused on
population-level performance measures. Phase I of the Population Health project endorsed measures addressing clinical
preventive services and immunizations, while Phase II sought to endorse healthy behaviors and broader population-level
measures, including those that are able to assess social, economic, and environmental determinants of health and
outcomes.

National Center for Minority Health and Health Disparities Releases "Roots of Health Inequity: A Web-Based Course for
the Public Health Workforce"

Association of County and City Health Officials (NACCHO), the National Center for Minority Health and Health Disparities

online learning environment from which to explore root causes of inequity in the distribution of disease, illness, and
death. It seeks to ground participants in the concepts and strategies that could lead to effective action. Each of the
five course units provide an in-depth look at a specific topic by using interactive maps and timelines, slideshows,
resource libraries, videos and interviews with practitioners.

There is no course fee, and interested individuals can begin downloading the course at any time.

ICD-10 Update

On August 24, 2012, HHS Secretary Sebelius announced a final rule [
http://www.hhs.gov/news/press/2012pres/08/20120824e.html ] that delays the compliance date for use of new International
Classification of Diseases, 10th Edition diagnosis and procedure codes, or ICD-10 until October 1, 2014. ICD-10 includes
codes for new procedures and diagnoses that provide enhanced information for quality improvement and payment purposes.
