We have issued a new Policy Information Notice (PIN) that establishes scope of project policy for HRSA-supported health centers that want to provide primary health care services to support transitions in care for justice-involved individuals reentering the community (JI-R). Visit HRSA’s website for the updated policy, including key details and other resources.
We are updating HRSA’s Electronic Handbooks to accommodate requests to add transitional care in carceral setting locations to your health center's scope of project. We anticipate this to be available by the end of the year; when it is, we will notify you in the Primary Health Care Digest newsletter. In the meantime, if you are interested in providing care to JI-R individuals in your community, review the PIN so you understand the requirements, including the documentation you will need to provide.
