The updated Patient-Centered Medical Home (PCMH) Healthy Equity Technical Assistance (TA) Symposium website now includes a detailed agenda for our two-day symposium. Your peers will present on topics as varied as “Integration of Diabetes and Oral Health,” “Leveraging a Health Information Exchange to Support Postpartum Care Planning,” and “Addressing Climate Emergencies through a Resilience Lens.”
