Ensure Your Scope of Project is Accurate
Your scope of project defines the activities Health Center Program funding can support. Maintaining an accurate scope of project is a requirement of the Health Center Program. Take time now to ensure your scope of project is accurate. Specifically, compare:
- The services you provide to what is on Form 5A.
- Your site addresses and hours of operation to what is on Form 5B.
You can use the Form 5A Accuracy Worksheet (PDF) and the Form 5B Accuracy Worksheet (PDF) to help you check if you need to change anything on these forms. We recently updated these worksheets to be independent self-assessments. They will guide you through analyzing the parts of each form.
Once you figure out what changes are needed, update your scope of project by submitting a Scope Adjustment or Change in Scope (CIS) request through HRSA’s Electronic Handbooks. HRSA’s standard timeframe for CIS review is within 60 days of receiving a complete submission, notwithstanding unique cases (for details, refer to PAL 2014-10 (PDF)).
Find policies, sample forms, and instructions on the Scope of Project Resources webpage. Questions? Submit them via the BPHC Contact Form (Program Monitoring H80 > Change in Scope > [select the best option for your question, e.g., if your question is about hours of operation, select Scope Verification/Adjustment]).
