Health Center Program 2035 (HCP 2035) is an effort to better position the Health Center Program for the future of health care in this country and maximize its impact in increasing access to care, improving health outcomes, and advancing health equity.
Using strategic foresight, this effort will involve anticipating future trends, challenges, and opportunities and using this information to guide future strategic decision-making. It is designed to be an inclusive process inviting perspectives and participation from diverse stakeholders.
With that in mind, we seek health center staff, patients, providers – as well as staff from Primary Care Associations, National Training and Technical Assistance Partners, and Health Center Controlled Networks – to participate in four planning sessions or other opportunities to share input.
We’ll use the feedback we receive to assist in developing a long-term strategic vision for the Health Center Program.
