BPHC will host a Patient-Centered Medical Home (PCMH) Health Equity Technical Assistance Symposium on Monday, January 31, and Tuesday, February 1. Participants will have the opportunity to hear about and discuss best practices and lessons learned from peers and experts in the fields of health equity, quality improvement, and accreditation/recognition.

Opportunity to Present
Health centers: The symposium includes an opportunity for you to present to your peers! We invite you to share data-driven quality improvement projects that improved health outcomes and/or reduced health disparities.
Each presenter will have 30 minutes: 15 for your presentation and the rest for discussion.

Health Center Presentation Tracks
The presentation portion of the symposium will have three tracks. We are especially interested in the priority areas listed below, relating to specific Uniform Data System (UDS) data and performance measures (refer to UDS Tables 6B - Quality of Care Measures and 7 - Health Outcomes and Disparities).
Track A: Behavioral Health
- Substance use disorder, especially opioid use disorder
- Screening for depression and follow-up plan
- Tobacco use: screening and cessation intervention
Track B: Health Management
- HIV: Linkage to care
- Hypertension:
- Statin therapy for the prevention and treatment of cardiovascular disease
- Ischemic vascular disease (IVD): use of aspirin or another antiplatelet
- Controlling high blood pressure
- Dental
- Dental sealants for children between 6-9 years
- Assessments
- Diabetes: Controlling Hemoglobin A1c
- Cancer Screenings
- Breast
- Cervical
- Colorectal
Track C: Maternal and Child Health
- Early entry prenatal care
- Childhood immunization status
- Weight assessment and counseling for nutrition and physical activity of children and adolescents
- Deliveries performed by health center’s providers
- Percent low and very low birthweight
