Vaccination Survey Questions Clarification
With last week’s survey (sent out on Friday, January 8), we added new questions asking how many staff members and patients have initiated and completed COVID-19 vaccination. We want to clarify what we mean:

We’re asking about the number of people who received the vaccination anywhere, not just at your health center. If a staff member or patient received a COVID-19 vaccination during the last week and you have a record of the immunization, please include them in your count.

Have questions about how to complete the survey? Visit the user guide where you can quickly “jump” to instructions related to a specific question.

Frequently Asked Questions (FAQ)
We recently added a Vaccines category to our COVID-19 FAQ webpage and continue to add questions to it. Other resources available include our coronavirus-related funding FAQ webpage and our COVID-19 Information for Health Centers and Partners webpage.
