Health Center Profile Updates

(2019 badges will be updated in the fall.) You can also see your health center’s project period length, end date, and any program conditions. EHBs Diabetes Action Plan User Guide for Health Centers
Health centers receiving an Operational Site Visit or diabetes targeted TA visit in 2019 will participate in a performance analysis review of the health center’s UDS diabetes measure. The health center will identify three performance improvement actions to reduce the percentage of patients with uncontrolled diabetes. HRSA will monitor the health center’s diabetes performance improvement actions using the EHBs Action Plan Module. Access the Diabetes Action Plan User Guide within the EHBs Help and Knowledge Base to learn the system steps.

HUD Policy Briefs for Health Centers
Corporation for Supportive Housing (CSH), a HRSA-funded National Cooperative Agreement (NCA), offers two new HUD policy briefs:

  • In August 2018, HUD launched “All Routes Home: Ending Homelessness in Rural America,” a strategy for building the capacity of rural communities to address the needs of persons experiencing homelessness. HUD Policy Brief on Rural Homelessness: Understanding the Impact and Potential for Health Centers outlines specific steps health centers serving rural areas can take to better connect their consumers experiencing homelessness or housing instability to housing and services.

  • Health centers can address social risk factors for their patients through partnerships and linkages to local housing and service resources. Health centers are increasingly working with HUD Continuums of Care (CoCs) to refer patients who are unstably housed or homeless to Coordinated Entry for services and housing interventions. One model of this is Rapid Re-housing, described in this HUD Policy Brief on Rapid Re-Housing: Understanding the Impact and Potential for Health Centers.
