The individual (ECF filer) filing a proof of claim is responsible for selecting (or adding) the correct creditor, including the name, notice address, and claim amount.
- The creditor’s information selected or entered by the ECF filer will be used for noticing, even if it is different from the name or address on the PDF claim form.
- If a creditor needs to change its name and/or address for noticing purposes, the creditor must file a motion and order.
List of Creditors/Mailing Matrix

Debtor’s counsel must ensure that:
- Creditors listed in the schedules are included on the matrix (PDF) filed in the case, and

Creditor’s information on the matrix (PDF file) is accurately entered into the CM/ECF system.
- Creditors’ information (name and address) is entered into CM/ECF uploading a .txt file. To ensure accurate noticing, debtor’s counsel should compare the creditors’ information listed in CM/ECF to the PDF version of the matrix filed in CM/ECF. To do this select [Query > Creditor]. If you find any discrepancies, please contact the clerk’s office.
