SUBJ: PROCEDURE FOR COAST GUARD HEALTHCARE SERVICE CHANGE REQUESTS
1. This ALCOAST provides procedural guidance for requested changes
to CG healthcare services including expansion, relocation,
reduction, or other alterations to the services delivered by CG
healthcare providers. Common examples include, but are not limited
to, expansion or changes in medical services provided; changes in
computer networking connections; establishment or modification of
relationships with nearby clinics, Military Treatment Facilities, or
network ancillary services; changes to radiology capabilities
including circuits and hardware; recalibration of equipment;
expanding or adding laboratory, pharmacy, radiology, or other
ancillary capabilities. In order for Military Health System (MHS)
GENESIS (the electronic health record) and healthcare equipment to
continue operating at full capability, software and/or hardware
configuration changes are required.
2. Change Request Procedure:
    a. When the need for a healthcare service change is identified,
the relevant Health Services Administrator (HSA) or designated
appointee shall address the need in memorandum format. HSA shall
include, at minimum, the following information: nature of the
request in comparison to the current situation, purpose,
justification, background, associated costs, number of assigned
patients and providers, status of local support, and projected time
frame of need.
    b. The HSA shall route a memo from the unit commanding officer to
HSWL SC (ma), with endorsement thru the cognizant Regional Practice
Manager (RPM), and a copy to COMDT (CG-1122), COMDT (CG-9333), and
HSWL SC Health Information Systems & Technology (HIST) Branch. The
endorsed memo should be emailed to SMB-HSWLSC-HealthcareService-
ChangeRequest@uscg.mil.
    c. HSWL SC will collect all requests for review on a quarterly
basis. HSWL SC will coordinate with the Health Services IT and
Infrastructure Configuration Control Board and COMDT (CG-1122),
COMDT (CG-9333), C5ISC, SILC, CGCYBER, DOL, FORCECOM, DCMS, DCO,
cognizant District and Area staff and others as needed.
    d. If the request is approved by HSWL SC, the cognizant RPM shall
designate a local project officer to lead the implementation. The
local project officer will coordinate with the applicable CG service
providers (C5ISC, HSWL, SILC, etc.) and local forces (purchasing,
scheduling, etc.) as needed to enact the change.
3. In lieu of the process defined in paragraph 2 of this ALCOAST,
certain changes with limited cost and minimal MHS GENESIS
reconfiguration requirements may be approved via the existing
CGFIXIT request process for MHS GENESIS changes. A change to clinic
address or phone number(s) without associated service expansion,
relocation, reduction, or alteration should be routed through
existing CGFIXIT request process. Questions regarding MHS GENESIS
reconfiguration requests in CGFIXIT can be emailed to
SMB-HSWLSC-HealthcareService-ChangeRequest@uscg.mil. All other
changes must follow the procedures in paragraph 2.
4. Corrections to errors in MHS GENESIS without changes to existing
medical location configurations should continue to be submitted in
CGFIXIT via established process.
5. Any emergent requests shall be initiated by contacting the
HSWL-MA-HIST Branch Chief or the POC in paragraph 6 of this ALCOAST.
