SUBJ: RESERVE INCAPACITATION REQUEST FORMS
1. This message announces the publication of the following official
Coast Guard forms into the forms library:
  a. CG-1001C U.S. Coast Guard Reserve Incapacitation Benefits Request
  b. CG-6300 Medical Support Allowance Billet Request
  c. CG-6300A Monthly Physician Report
2. Effective 01 Jun 2019, the use of these forms will be mandatory on
any new reserve incapacitation benefit claims.  Pre-existing cases, on next
monthly submission of physician reports must use the new form(s) immediately.
Cases routed prior to the 01 Jun 2019 deadline will be accepted, but any
additional requested information must be submitted using the new forms.
3. Completed forms shall be submitted to e-mail:
