SUBJ: GENDER DYSPHORIA MEDICAL POLICY UPDATES
A. Secretary of Defense Directive-type Memorandum (DTM) 19-004,
“Military Service by Transgender Persons and Persons with Gender
Dysphoria,” March 12, 2019
B. Coast Guard Medical Manual, COMDTINST M6000.1 (series)

D. Military Transgender Service, COMDTINST M1000.13 (series)
1. This ACN updates REF (A) accession and retention standards for military Service members
(Active Duty and Reserve) who have been diagnosed with gender dysphoria.
2. The following changes to REF (B) are effective immediately:
   a. 5.A.14.a.- Gender Identity Disorders replaced by.
   b. 5.A.22. - Gender Dysphoria (302.85). This disorder is presumptively disqualifying for
appointment, enlistment, induction or retention. Unless exempt per Military Transgender Service,
REF (D), members diagnosed with gender dysphoria shall be processed in accordance with Military
Separations, REF (C), Physical Disability Evaluation System, COMDTINST M1850.2 (series) and Military
Transgender Service, REF (D), as applicable. Nonexempt Service members will be provided necessary
care consistent with Section 1074 of Title 10, United States Code and the July 29, 2016 Assistant
Secretary of Defense for Health Affairs Memorandum for as long as the individual remains a Service
member as set forth in a medical treatment plan developed with the military medical provider and
provided to the CO/OIC.
3. These policy changes will be reflected in the next update of REF (B).
