SUBJ:  UPDATED MILITARY TRANSGENDER SERVICE POLICY AND TIME EXTENSIONS FOR DIAGNOSIS

Persons and Persons with Gender Dysphoria”
1. This ACN announces the cancelation of REF (A) and the promulgation of REF (B).
2. This manual incorporates provisions of REF (C).
3. Major Changes. 
   a. Service members who accessed in their preferred gender or recieved a diagnosis from, or had
such diagnosis confirmed by, a military medical provider before 12 APR 2019 will be allowed to
continue serving in the military pursuant to the policies and procedures in effect prior to
12 APR 2019.
   b. On or after 12 APR 2019, Service members diagnosed with gender dysphoria may be retained
if they do not require a gender transition to protect their health, as certified by a military
medical provider, and are willing and able to serve in their biological sex and comply with all
standards applicable to their biological sex.
   c. Persons with a history of gender dysphoria who do not have a history of cross-sex hormone
therapy or any gender affirming surgeries may join the Coast Guard if they can demonstrate a
36-month period of stability in their biological sex. An applicant with a history of cross-sex
hormone therapy, sex reassignment or genital reconstruction surgery is presumptively disqualified
from service.
4. This manual also provides updated policies for retention, accession, waivers and separation
of persons with gender dysphoria.
5. Any military member of the Coast Guard (Active or Reserve) serving on or before 11APR2019 and
who believes that he or she is gender dysphoric, but as a direct result of a deployment, underway
operation, or temporary assignment to a remote location without reasonable access to a military
medical facility, was unable to obtain a diagnosis of gender dysphoria from a military medical
provider prior to 12 APR 2019, has 30 days from the completion of such deployment, underway
operation, or return from temporary remote assignment to obtain an initial diagnosis. Any member
