1. To ensure Individual Medical Readiness (IMR) and to ensure Coast Guard (CG)
service members (SMs) receive all recommended Clinical Preventive Services,
REFs (A-B) require all CG military personnel to complete a PHA at least once every
12 months. REF (C) promulgated REF (B), which implemented the new All Services PHA
for the CG.
2.  It’s been 6 months since the new PHA has been implemented, however only 25% of
Coast Guard SMs have completed the new assessment.   Coast Guard SMs should not wait
until Jan or Feb 2019 to complete the new PHA because it will result in a delayed
ability to complete PHAs.
3. In order to improve PHA completion rates, effective 1 Sep 18 through 1 Mar 19,
CG SMs assigned to CG clinics may use the Reserve Health Readiness Program (RHRP) to
complete their PHAs. This gives these CG SMs another option to accomplish their PHA.
CG SMs who are on aviation or dive status must complete their PHA at a CG or
Department of Defense (DoD) clinic.
4. Once CG SMs complete and certify their online PHA questionnaires
(https://data.nmcphc.med.navy.mil/pha/), they must contact their CG clinic and
request either a PHA appointment at the clinic or a PHA appointment through the RHRP.
If a SM selects the RHRP option, the RHRP contractor (Logistics Health, Incorporated)
will contact the SM to arrange the appointment, generally within 6 business days.
5. CG SMs who are part of the Selected Reserves or SMs who receive their primary
care with Tricare network providers must continue to use RHRP for PHA completion.
These SMs must contact their regional Health Services Technician (HS) after
completing their online PHA questionnaire. Contact information for regional HSs by

6. CG SMs who complete their PHAs at DoD Military Treatment Facilities (MTFs) may
continue to do so, according to the MTF’s procedures.
7. REF (D) cancelled COMDT COGARD WASHINGTON DC 160816 AUG 18/ALCOAST 286/18; CG SMs
who completed the old PHA no longer need to complete a standalone Mental Health
Assessment. Instead, the requirements of REF (E) will be met through the new
All Services PHA.
8. Units are encouraged to use RHRP Group Events to accomplish PHAs and to meet
other IMR requirements. HSC Athan Anderson (athan.anderson@uscg.mil) is the CG POC
