1. CG personnel involved in hurricane response operations may experience injuries
and/or illnesses that could have long term health effects and/or fitness for duty
implications. To ensure that such events are captured in the Service Treatment
Records (STRs) of CG personnel, and to ensure that CG personnel receive appropriate
medical care related to such events, all CG personnel involved in hurricane response
operations must complete a post-response health assessment.
2. REF (A) directs all CG personnel who participated in a 2017 hurricane response to
complete the Significant Event Acute Exposure Tracking Tool (SEAETT), which is found
at https://hswl.uscg.mil/kseomsep/AcuteExposure/index_omsep.asp. 
3. REF (B), Chapter 2 Section A.1.d.(3)(a), requires that SELRES personnel who have
active duty orders for more than 30 days also complete a post-activation Periodic
Health Assessment (PHA) and a Report of Medical Assessment, Form DD 2697 within 10 days
of leaving active duty. However, so as not to duplicate the amount of information being
requested from SELRES members, only the completion of the SEAETT is required for 2017
SELRES hurricane responders. The post-activation PHA and Form DD 2697 are still
required for all other CG SELRES personnel with active duty orders for more than 30 days.
4. The annual PHA for Individual Medical Readiness purposes will still be required for
all CG military personnel per REF (B).
