Announcement of Revisions to RD Civil Rights Forms and Posters

Rural Development’s Office of Multifamily Housing is providing this notification to applicants, borrowers, management agents and lenders regarding recent changes to various Rural Development (RD) forms and posters. 

On June 13, 2025, RD’s Civil Rights Office announced revisions to the forms and posters listed in the tables below:

Revised – Current Forms

Note. If you receive a message that the document you are trying to load requires Adobe Reader 8 or higher, follow these steps:

  • Save the RD 400 – 4 to your computer
  • Open Adobe Acrobat
  • Click Menu>Open
  • Click on the RD 400 – 4 pdf file that you saved on your computer
