The MCP Health Equity Plan (HEP) template will be available in the MCP Participant Portal beginning tomorrow, October 1. All participants must submit their initial plan for review by Thursday, October 31. CMS has created the following resources to support participants’ HEP development:
- The MCP HEP Guide may be used to develop your initial HEP submission and includes the questions you will complete in the portal. Step-by-step explanations and recommended resources are included throughout to help you develop a plan of action for your organization.
- The accompanying Appendix contains key terms, available data sources, and recommended strategies to further your HEP development.
- The on-demand Milestone Videos on MCP Connect walk through the four major milestones in HEP development. Note: To view these videos, log into MCP Connect, select “Libraries,” then click on the MCP Connect folder followed by the Advance Health Equity folder to download them.
- The Portal User Manual is available in the MCP Participant Portal under Helpful Links for any issues submitting your HEP.
The purpose of the HEP is to collect and use data to identify disparities in outcomes in patient populations, set goal outcomes, implement initiatives to reduce these disparities, and measure these changes over the course of the model. Note: Achieving or not achieving the goal outcomes will not be used to measure your organization’s performance under the model.
