Upcoming All Practice and CTO Office Hour Rescheduled |
The next Quarterly All Practice and CTO Office Hour has been rescheduled from Thursday, October 7, 2021, to Thursday, October 28, 2021. If you registered for the October 7, 2021 date, you do not need to register again and may use the same event link provided via email at the time of your registration. This is a great opportunity for practices to learn more about upcoming deadlines, review guidance, and participate in a Q&A forum. Upcoming session topics to be announced. COVID-19 Vaccination Campaign Updates
The FDA and CDC authorized a 3rd dose of mRNA COVID-19 vaccine for moderately or severely immunocompromised patients.
The MDPCP PMO Team has created the following guide to support your practice in advising patients and recommending administration of a 3rd dose of COVID-19 vaccine for certain immunocompromised patients:

In the guide, you will find:
- Information on which patients are eligible for a 3rd dose
- Key points on the indication for a 3rd dose
- Implementation considerations
Use this guide to reach out to your eligible patients and recommend vaccination with a 3rd dose appointment.
Continued outreach to unvaccinated patients:
Now more than ever, we must increase efforts to vaccinate our most vulnerable populations. Your practice can aid in this effort by calling your unvaccinated patients, asking why they remain unvaccinated, and assisting them in scheduling a vaccine appointment if accepted.
To aid you in this outreach effort, the PMO has created the New COVID-19 Vaccine Outreach Resources:
Please use these resources to outreach to your unvaccinated patients, support them in getting vaccinated, and getting us closer to ending this pandemic.

Open Enrollment for MDPCP CTO Partnership
The open enrollment window for MDPCP CTO partnership selection is now open, and will run until Friday, September 10th. Please use the online form found here to indicate your preference for a CTO partner and to select your desired Care Management Fee (CMF) split for 2022.
We request that all practices complete this form even if you are not making changes so that we can officially confirm your choice for next year.
- If you do not wish to partner with a CTO in 2022, please select "No CTO Partner" for all three questions.
- If you are the practice contact for multiple locations, please complete this form for each MDPCP site.
- Any practices that do not respond by the deadline will default to their existing CTO and CMF split. CMS and the PMO will reconcile each practice's selection with each CTO's service area and capacity, and will announce final pairing decisions later this fall.

Advanced Primary Care Leadership Academy Postponed
The Advanced Primary Care Leadership Academy has been postponed until further notice due to rising concerns about limitations on indoor gatherings. An in-person format is most conducive to learning and leadership development and we look forward to seeing you at an in-person Leadership Academy soon.

COVID-19 Update Webinars
In light of the continuing challenge that COVID-19 poses to public health in Maryland and nationally, the MDPCP PMO will resume hosting biweekly COVID-19 update webinars for all Primary Care Providers. These webinars will allow providers to review current guidance, provide additional information on important activities primary care offices should be taking for the health and safety of Marylanders, and answer additional questions. Topics will include vaccine program highlights, the future of the vaccine program, COVID-19 variants, testing, long COVID-19, monoclonal antibodies, and the long-term outlook for the pandemic. State of the MDPCP All Practice Call; 10/1, 12-1pm
Join the MDPCP Program Management Office for their first All-Practice Call. This meeting will discuss the performance of the MDPCP program in key metrics and high-level strategy for the coming year. All practices should join to understand the present and future state of the MDPCP, and how their practice fits in. Register above.

August Reports
In August, CRISP, in collaboration with hMetrix released the following report updates:
Maryland Primary Care Program (MDPCP) Reports:
- The Population Characteristics and Utilization Report have been renamed "Health Equities by Demographics Report."
- The Beneficiary Details drill through view has been enhanced with functionality to select specific columns of interest as well as filter and sort on column values. Please refer to the MDPCP Reports User Guide for more information.
- The logic used in the PQI-Like Events report and the Pre-AH model, has been updated to AHRQ PQI version 2020. PQI #10: Dehydration has been removed from the latest measure set.
- Data Q1 2021 are available in the AHU/EDU Report. For practices new to MDPCP in 2021, this will be their first AHU/EDU data in this report.
MDPCP Performance Reports:
- Filters have been added to the MDPCP Retrospective Performance Sandbox to select specific CTOs or, as a user's permissions allow, specific practices to populate the report. Please refer to the MDPCP Performance Reports User Guide for more information.

Track 2 Requirements
The following CRISP Requirements need to be completed for your attributed beneficiaries, at a minimum, to meet requirements for Track 2 transition. Remember to keep a record of these as you may be subject to validation and audit.
- Care alerts for attributed beneficiaries
- ENS Panel is up to date (must be within the last 90 days)
- Review your Pre-AH Avoidable Hospital events tool and follow up with your high-risk beneficiaries monthly.

MDPCP Connect was upgraded to Single Sign-On (SSO) in November 2020. If you don’t have a Connect account or still need to complete SSO, we would love to have you on the site. Below, we have included some instructions and support.

- Be sure to use the new URL: https://app.innovation.cms.gov/CMMIConnect
- Click “Existing User Verification". If you don’t have them already, you will need to create IDM credentials to complete SSO. The verification option will guide you through this process.
- As a reminder, you will already have IDM account credentials if you have previously logged into the CMS portal.
