Profion PCR dilynol i’r rheini sy’n cael canlyniad positif i brawf LFT

Welsh Government logo

Annwyl Gyfeillion,

Rydyn ni’n ysgrifennu atoch chi i roi gwybod am newid pwysig i gyngor Llywodraeth Cymru i’r rheini sydd wedi cael canlyniad positif i brawf llif unffordd (LFT).

Mae’r galw am brofion PCR ym mhob rhan o’r DU wedi cynyddu’n aruthrol sy’n golygu bod Asiantaeth Diogelwch Iechyd y DU ar brydiau’n gorfod cyfyngu ar nifer y profion y mae’n eu cynnal i amddiffyn amser prosesu i gael canlyniadau. Hefyd, mae llawer o Fyrddau Iechyd Lleol sy’n cynnal Unedau Profi Cymunedol wedi cael eu boddi gan nifer y ceisiadau i gynnal profion PCR gan staff Iechyd a Gofal Cymdeithasol ledled Cymru.

Rydyn ni felly, gyda thair gwlad arall y DU, wedi cytuno i newid ein cyngor i bobl heb symptomau sy’n profi’n bositif i brawf LFT. O 6 Ionawr 2022, nid oes angen i’r bobl hyn gael prawf PCR dilynol. Mae’r datganiad ysgrifenedig ar gael drwy’r ddolen hon Datganiad Ysgrifenedig: Blaenoriaethu Profion PCR (5 Ionawr 2022)

Cafodd y penderfyniad hwn ei wneud ar sail y dystiolaeth sy’n awgrymu wrth i achosion o’r clefyd gynyddu, bod canlyniadau profion LFT a PCR yn mynd yn fwy tebyg a bod y posibilrwydd y gallai’r prawf LFT roi canlyniad positif ffug yn lleihau sy’n golygu bod llai o werth cadarnhau canlyniadau profion LFT trwy gynnal profion PCR.

Mae’r newid hwn yn golygu ei bod yn bwysicach nag erioed bod pob canlyniad LFT (positif/negyddol/annilys) yn cael ei gofnodi ar wefan cofnodi profion llif unffordd (LFT) COVID-19 Llywodraeth y DU. Mae canlyniadau positif yn bwydo’r system olrhain cysylltiadau i gyflymu’r broses olrhain cysylltiadau a’n helpu i gysylltu â phobl all fod yn heintus yn gyflym.

Mae ein polisi yn parhau na ddylai unrhyw un sydd wedi profi’n bositif drwy brawf PCR gymryd prawf PCR arall o fewn 90 diwrnod oni bai ei f/bod yn datblygu symptomau COVID-19 craidd newydd. Fodd bynnag, hoffem gymryd y cyfle i gadarnhau nad yw’r canllawiau hyn yn cynnwys profion llif unffordd a ddylai gael eu defnyddio ar gyfer profi cyn mynd i’r gwaith, profi ar ddiwrnod 6 & 7 o unrhyw gyfnod hunanynysu ac ar gyfer polisïau profi staff sy’n gysylltiadau, hyd yn oed os oes haint blaenorol.

Yn ogystal â'r newid uchod, mae'r cyngor ar gyfer yr holl gysylltiadau sydd heb eu brechu wedi newid, a dylai'r unigolion hynny bellach gymryd prawf llif unffordd ar ddiwrnod dau a diwrnod wyth yn hytrach na phrofion PCR yn ystod eu cyfnod ynysu o 10 diwrnod.

Rydyn ni’n gweithio gyda’n partneriaid ar y newid hwn er mwyn gallu ei roi ar waith cyn gynted â phosibl.

Yn gywir

Prof Chris Jones

Archebu pecynnau prawf llif unffordd

Cyn y Nadolig, fe wnaethom hysbysu holl ddarparwyr gofal cymdeithasol am y newid o brofion llif unffordd ddwywaith yr wythnos i brofion llif unffordd 'dyddiol' ar gyfer staff gofal cymdeithasol rheng flaen ac fe wnaethom annog pawb i wneud y mwyaf o'u harchebion cyn y Nadolig er mwyn sicrhau cyflenwadau digonol. Fe wnaethom hefyd ddarparu archebion swmp ar frys o becynnau profi i Awdurdodau Lleol fel bod modd i chi gysylltu â nhw am gyflenwad os oeddech yn rhedeg yn fyr dros gyfnod y Nadolig.  Gofynnwn i chi barhau i wneud hyn nes i'ch archebion nesaf gyrraedd.

Rydym bellach wedi derbyn hysbysiad, ar yr amod nad oes unrhyw broblemau technegol, y bydd y newidiadau i system archebu Salesforce i newid darparwyr gofal cymdeithasol i becynnau Innova 25 o brofion llif unffordd ar gyfer profi staff 'dyddiol' yn fyw ar y 6ed o Ionawr ar gyfer y lleoliadau canlynol:

Welsh Services

Mae hyn yn golygu bydd modd i’r lleoliadau uchod ddechrau ail-archebu nifer uwch o brofion drwy eu sianeli arferol o heddiw ymlaen - (1 pecyn o 25 o brofion llif unffordd fesul aelod o staff bob 21 diwrnod).

Fodd bynnag, nid oes disgwyl i’r newid hwn fod yn fyw ar gyfer Cartrefi Gofal i Oedolion tan y 12fed o Ionawr, sy'n ddyddiad diweddarach oherwydd gofynion ychwanegol i rannu'r archebion ar gyfer staff a fydd ar becynnau o 25 ac ar gyfer ymwelwyr y darperir pecynnau o 7 ar eu cyfer, unwaith eto mae hyn ar yr amod nad oes unrhyw broblemau technegol.

Rydym yn aros am ddyddiad ar gyfer y newid i Gartrefi Gofal Plant gan nad ydynt wedi cael profion llif unffordd ar gyfer profion staff o'r blaen ond rydym yn rhagweld y bydd yn fuan.

Os byddwch yn nodi eich bod yn debygol o fod â diffyg profion yn y cyfamser, cysylltwch ag arweinydd profi eich Awdurdod Lleol a fydd yn gweld beth ellid gwneud i sicrhau cyflenwad o becynnau prawf i chi.

Amseroedd Prosesu Profion PCR

Mae ymddangosiad yr amrywiolyn Omicron wedi creu galw digynsail am brofion a phrinder staff mewn labordai. Mae hyn yn achosi oedi wrth brosesu rhai profion PCR

Mae Profi ac Olrhain y GIG yn gweithio'n galed i brosesu'r ôl-groniad presennol o brofion ac maent yn cymryd camau i leihau'r risg y bydd samplau'n annilys oherwydd oedi prosesu.

Darperir canlyniadau ar sail unigol a dylai unrhyw un sy'n cael canlyniad prawf annilys gymryd prawf arall.

Dylai staff sydd â symptomau barhau i hunanynysu wrth aros am ganlyniad PCR, a dylai staff sy'n gyswllt ag achos cadarnhaol barhau i aros am ganlyniad prawf PCR negyddol cyn dychwelyd i'r gwaith.

Mae 119 yn derbyn nifer uchel iawn o alwadau oherwydd y mater hwn. Rydym yn cydnabod y pryder mae’r fath hwn o oedi yn achosi mewn lleoliadau gofal; mae Profi ac Olrhain y GIG yn gweithio'n galed i leihau amseroedd prosesu.

Follow up PCR tests for those who test positive using a LFT

Welsh Government logo

Dear Colleagues,

We are writing to inform you of an important update to Welsh Government advice
which will affect the actions of those that have tested positive via lateral flow tests
(LFTs).

Demand for PCR testing has reached unprecedented levels across the UK meaning that at times the UK Health Security Agency (UKHSA) has had to constrain bookings in order to preserve the turnaround times for results. In addition many Local Health Board run Coronavirus Testing Units (CTUs) have been overwhelmed with PCR bookings from Health and Social Care staff across Wales.

In response we have agreed, alongside the other three nations of the UK, that from 6 January 2022 changes will be made to the advice provided to people without symptoms who have tested positive on a LFT meaning they will no longer be advised to take a follow up PCR test. The written statement can be found via this link Written Statement: Prioritising PCR Testing (5 January 2022)

We take this decision in the knowledge that evidence suggests that as prevalence of disease increases LFTs and PCRs have similar positive predictive values and the possibility of LFT false positives decreases meaning the value of confirming LFT results with a PCR tests lessens.

The change means it is now more important than ever that all LFT results (positive/negative/invalid) are reported via Report a COVID-19 rapid lateral flow test result.  Positive results that are reported flow into the contact tracing system which speeds up the contact tracing process and helps to identify and contact potentially infectious people quickly.

Our longstanding policy has been that someone who has previously tested positive via PCR should not take another PCR test for 90 days unless they develop new core symptoms of COVID-19. However I wanted to take this opportunity to clarify that this guidance does not apply to LFTs which should be used regardless of previous infection for regular testing before work, testing on day 6 & 7 of any isolation period and in line with the testing policy for staff who are contacts.

In addition to the change above the advice for all unvaccinated contacts has
changed and these individuals should now take a lateral flow test on day two and
day eight instead of PCR tests during their 10 day isolation period.

We are working with our partners to support the implementation of this change in order for operational changes to be made as soon as possible.

Yours sincerely

Prof Chris Jones

Ordering LFD test kits

Before Christmas we notified all social care providers of the change from twice weekly LFD testing to ‘daily’ LFD testing for frontline social care staff and we urged everyone to maximise their pre Christmas orders to ensure adequate supplies.  We also provided Local Authorities with emergency bulk orders of kits so that you could contact them for supplies should you find yourselves running short over the Christmas period.  Please continue to do this until your next orders arrive.

We have now received notification that, providing there are no technical issues, the changes to the Salesforce ordering system to switch social care providers over to the Innova 25’s packs of LFDs for ‘daily’ staff testing is due to go live on the 6 January for the following settings:

English Services

This means that the above settings can start to reorder larger numbers of tests through their usual channels as from today- (1 pack of 25 LFD tests per member of staff every 21 days).

However this change is not due to go live for Adult Care Homes until the 12 January, which is a later date due to additional requirements to split the ordering for staff who will be on packs of 25’s and for visitors for whom packs of 7 will be supplied, again this is providing there are no technical issues.

We await a date for the change to Children’s care homes ordering as they have not previously had LFDs for staff testing but we anticipate it will be soon.

If you identify that you are likely to have a shortfall of tests in the meantime please contact your Local Authority testing lead who will see what they can do to supply you with test kits.

PCR Test Turnaround Times

The emergence of the Omicron variant has created unprecedented demand for testing and staff shortages in laboratories. This is causing delays in the processing of some PCR tests

NHS Test and Trace is working hard to process the current backlog of tests and are taking steps to minimise the risk of samples being voided because of processing delays.

Results are provided on an individual basis and anyone who receives a void test result should take another test.

Staff who are symptomatic should continue to self-isolate while waiting for a PCR result, and staff who are contacts of a positive case should continue to wait for a negative PCR test result before returning to work.

119 is receiving extremely high call volumes because of this issue. We recognise the concern that delays of these kind cause in care settings; NHS Test and Trace has assured us that they are working hard to reduce turnaround times.
