In Gwent during ‘Dementia Action week’ (17 – 23 May 2021), Torfaen Council would like to promote our part in the National Bevan Exemplar project, ‘Get There Together’.
The project is led by Dr Natalie Elliott, National Consultant AHP Lead for Dementia, at Cardiff and Vale University Health Board.
The council have been working in collaboration to develop video and leaflet resources for people living with dementia and their carers to support social and functional activity recovery, as COVID-19 restrictions are eased.
The 'Get There Together' project presented the opporunity to work with a number of large high street business such as Costa Coffee, WH Smith and Specsavers, including super markets such as Asda, Tesco and Sainsbury’s. We have also worked with Newport Transport and Stagecoach to develop clips and other organisations such as carer’s hubs, GP surgeries, pharmacies and libraries.
The films are around two minutes long and have been especially designed for each venue to include voiceovers, pictures and text to help people who wish to familiarise themselves with changes due to Covid 19.
The videos enable people to see new safety measures, such as one-way systems, social distancing signs and clear screens, which could be confusing when first encountered. The video clips have been produced alongside people living with dementia and their carers, who identified where clips were needed and what content would be helpful and supportive.
Over 30 clips have been developed so far across Gwent and were extremely fortunate to have some fantastic Occupational Therapy students from Cardiff University, develop most of those during their placement at ABUHB.
In Gwent the project is coordinated and led by the Gwent Regional Partnership Team and the steering group consists of partners from the 5 Local Authorities including elected members, ABUHB, Alzheimer’s Society, Welsh Ambulance Service, Gwent Police, Social Care Wales, Bridges, Breaking Barriers Community Arts, Cwmbran Centre, Bridges Centre Monmouth and National Museum Wales.
Newport Councillor Paul Cockeram, who is an active member of the Gwent sub group says, “As Chairman of the Regional Partnership Board, we consider Dementia a high priority. This has resulted in major investments through our ICF programme, which has led to positive outcomes for people living with dementia and their carers.”
Caerphilly Council Councillor Carol Andrews, who is also an active member of the group says, “As Dementia Champion for CCBC it is really important to me raise awareness about people living well with Dementia. Following the easing of restrictions everyone is naturally hesitant about returning to normal and this project helps to put minds at ease, with simple to follow video clips. It has been an honour to be involved and help to create a fantastic resource for the people of Wales especially in Gwent”.

Yn ystod ‘Wythnos Gweithredu Dementia’ (17 – 23 Mai 2021) yng Ngwent, hoffai Cyngor Torfaen hyrwyddo’n rhan yn y Cynllun Enghreifftiol Cenedlaethol ‘Mentro Gyda’n Gilydd’.
Arweinir y cynllun gan y Dr. Natalie Elliott, Ymgynghorydd Cenedlaethol Arweinydd PPI ar gyfer Dementia, gyda Bwrdd Iechyd Prifysgol Caerdydd a’r Fro.
Mae’r cyngor wedi bod yn cydweithio i ddatblygu adnoddau fideo a thaflenni i bobl sy’n byw gyda dementia a’u gofalwyr i gefnogi adfer gweithgaredd cymdeithasol a defnyddiol, wrth i gyfyngiadau COVID-19 gael eu llacio.
Cyflwynodd cynllun 'Mentro Gyda’n Gilydd' y cyfle i weithio gyda nifer o fusnesau mawr y stryd fawr fel Costa Coffee, WH Smith a Specsavers, gan gynnwys archfarchnadoedd fel Asda, Tesco a Sainsbury. Rydym wedi gweithio hefyd gyda Thrafnidiaeth Casnewydd a Stagecoach i ddatblygu clipiau a sefydliadau eraill fel hybiau gofalwyr, meddygfeydd teulu, fferyllfeydd a llyfrgelloedd.
Mae’r ffilmiau rhyw ddwy funud o hyd ac wedi eu cynllunio’n arbennig ar gyfer pob lleoliad i gynnwys trosleisio, lluniau a thestun i helpu pobl sydd am gyfarwyddo gyda’r newidiadau o ganlyniad i Covid 19.
Mae’r fideos yn galluogi pobl i weld mesurau diogelwch newydd, fel systemau un ffordd, arwyddion cadw pellter cymdeithasol a sgriniau clir, a allai peri dryswch ar y cychwyn. Mae’r clipiau fideo wedi eu cynhyrchu ochr yn ochr â phobl sy’n byw gyda dementia a’u gofalwyr, a nododd ble roedd angen clipiau a pha gynnwys byddai o gymorth ac yn gefnogol.
Mae dros 30 o glipiau wedi eu datblygu hyd yn hyn ar draws Gwent a roeddem yn ffodus iawn o gael myfyrwyr Therapi Galwedigaethol anhygoel o Brifysgol Caerdydd i ddatblygu’r rhan fwyaf o’r rheiny yn ystod eu lleoliadau gyda BIPAB.
Cydlynir y cynllun yng Ngwent gan Dîm Partneriaeth Ranbarthol Gwent ac mae’r pwyllgor llywio’n cynnwys partneriaid o’r 5 Awdurdod Lleol gan gynnwys aelodau etholedig, BIPAB, Cymdeithas Alzheimer, Gwasanaeth Ambiwlans Cymru, Heddlu Gwent, Gofal Cymdeithasol Cymru, Pontydd, Breaking Barriers Community Arts, Canolfan Cwmbrân, Canolfan Pontydd Mynwy a’r Amgueddfa Genedlaethol.
Dywed Cynghorydd Casnewydd, Paul Cockeram, sy’n aelod o is-grŵp Gwent, “Fel Cadeirydd y Bwrdd Partneriaeth Ranbarthol, rydym yn ystyried bod Dementia’n flaenoriaeth uchel. Mae hyn wedi arwain at fuddsoddiadau sylweddol trwy ein rhaglen Cronfa Gofal Integredig, sydd wedi arwain at ddeilliannau cadarnhaol i bobl sy’n byw gyda dementia a’u gofalwyr.”
Dywed Cynghorydd Caerffili, Carol Andrews, sydd hefyd yn aelod o’r grŵp, “Fel hyrwyddwr Dementia CBSC mae’n hynod o bwysig i mi ein bod yn codi ymwybyddiaeth am bobl sy’n byw gyda Dementia.
Ar ôl llacio cyfyngiadau, mae pawb yn betrusgar am ddychwelyd at normal ac mae’r prosiect yma’n gymorth i dawelu meddyliau, gyda chlipiau fideo hawdd eu dilyn. Mae hi wedi bod yn anrhydedd cymryd rhan a helpu i greu adnodd anhygoel i bobl Cymru, yn enwedig yng Ngwent”.
