Better mental health care for Barnet, Camden, Enfield, Haringey and Islington

Giving service users greater control of their care plan |
|
We are rolling out a changed approach to care planning, to ensure that what service users say are their priorities and concerns, are at the heart of their care plans. These co-produced plans will help clinicians and teams support service users more holistically around their medical care, psychological well being and social aspects of their lives.
This change follows an announcement by NHS England about care-planning for people accessing community-based mental health services. In the new guidance NHS England said every adult accessing community-based care should
co-produce a holistic personalised care and support plan from 2022. They should also be provided with the name of a key worker they can contact if their needs change. Our new care planning approach, called DIALOG+, makes it much easier to co-produce a personalised care and support plan with people. This will replace the Care Programme Approach (CPA) with research showing that DIALOG+ significantly improves quality of life and care experience.
Pilots are underway now and clinicians are rolling out the use of DIALOG+, and explaining to people how it will affect them. Service users are asked to rate different aspects of their life, including their relationships, their health, their accommodation, their medication etc.
They then work with the community team to improve the areas they are least happy with.

To mark LGBT+ History Month, we look at a service committed to helping improving the lives of LGBT+ people
For most of his life Dave kept his sexuality hidden. He didn’t feel able to tell people openly that he was gay and believes the stress that caused him affected his mental health.
While receiving treatment for a mental health condition, Dave was told about Outcome, Islington Mind's LGBTIQ+ specialist service. The mental health charity is one of the organisations working in partnership to deliver holistic, wraparound care in the communities where people live.

Dave (not his real name to protect his anonymity) tells his story:
“I was in hospital and someone mentioned Islington Outcome, and I thought I would give it a try. I immediately felt I could be myself, everyone was friendly and it’s been great.
“It’s really helped my mental health conditions. By coming here, it made me feel comfortable with my sexuality. Before that it was very hard. At first I was a bit nervous, but I made friends and began socialising, so it has been very helpful to me.
“I have a lot of mental health issues and coming here has really helped with that, made it easier for me to settle down. I can come here and just be myself. I help in the kitchens and I’ve done other classes like the art workshops.
“It helped relieve my mental health condition because I could relax and the staff and volunteers and other people coming here were all supportive. I made a lot of gay friends that I never had before.
“They also offer support with other things, filling in forms and things. Now, if I fill in a form and it asks about sexuality, I put I’m gay, and I’m proud of it.

Outcome will be hosting an online celebration for LGBT+ History Month on Tuesday, 22 February, offering activity tasters, such as yoga, art, creative writing, and themed performances from clients, a film room, online museum tours, and more. You can talk with clients, volunteers, and staff to learn more about our weekly wellbeing and mental health support service for the LGBTIQ+ community across London.

Structural changes to our community teams in Barnet and Haringey |
|
Community teams in Barnet and Haringey are being restructured as part of our £25 million, three-year Community Mental Health Transformation Programme.
These changes are being made so that the new Core Teams are aligned with the Primary Care Networks (PCNs), enabling us to work more closely with GP practices to bring greater consistency of high-quality care across the boroughs.

The new Core Teams are multidisciplinary teams, with clinical and non-clinical staff, that will continue to support working-age adults with complex mental health, social care and psychological needs. This will include mental health assessments; medical input, psychological therapies, support with social care needs, physical health assessments, follow-up care and practical help with social factors that can impact mental health for example advice around housing, accessing benefits and getting into work.
The Core Teams will work closely with teams providing psychological therapies, and the links between the Core Teams and other BEH services providing more intensive help are being strengthened.
No action is needed by service users or clinical professionals; these changes will be administered by programme teams.
Changes in Barnet
The Barnet East and Barnet North Locality Teams are merging to form the Barnet North Adult Core Community Mental Health Team from Tuesday 1 February, 2022.
As before, services will continue to be provided at the Springwell Centre at Barnet General Hospital and are available to adults registered with practices in Barnet Primary Care Network 2 and 3 with a referral from a primary care professional, internal professional teams and external organisations.

Changes in Haringey
The four locality teams in Haringey - West, Central, North East and South East; will be reorganising to form three Core Community Teams: East Core Community Team, the Central Core Community Team and the West Core Community Team.
We are in the process of recruiting around 40 additional members of staff into the three, new Haringey Core Teams. As well as continuing to provide services from St Ann’s Hospital, they will provide services from a range of other locations in the community and will ultimately be based in the community with primary care colleagues.
The reorganisation will start in February 2022 beginning with the East Team and the North East and Welbourne PCNs. Please keep checking the BEH website for more details.
