us at the Carers Forum
Carers
are invited to come along to the next Havering Carers Forum
on Tuesday 2 August at Romford YMCA, 10.30-2.30pm. Coming along will give you
the chance to hear about the draft Joint Havering Carers Strategy, meet with
the organisation Voiceability who provide independent advocacy for carers. You will also have the oportunity pick
up information on local services and to have lunch and talk to other carers and
representatives from Havering Council and Havering Clinical Commissioning
Group. Book

Launch
of the Shared Lives service in Havering
Shared
Lives offers adults with social care support needs the chance to receive the
care and support they need in a family home with a Shared Lives Carer in the
local community. People live in the homes of carers who have been recruited,
trained and approved for this purpose. Arrangements can be short term, lasting
anything from one night to several weeks, enabling both the person and their
regular carers to have a break. Or they can be more long term, providing the
opportunity for the person receiving support to live as part of the family.
The
service is looking to recruit more people to become Shared Lives carers.Anyone who has
a spare room, and wants to share their lives with someone who needs some extra
support, can apply. The team will fully support
you through the application process, offer on-going training and support and conduct all the necessary checks.
Shared Lives Carers are
paid for the support they provide individuals. You do not need to have lots of experience
or qualifications, it is more important that you respect and value people as
individuals and are willing to learn and attend training as necessary.
The
Council, in partnership with the British Red Cross, has re-launched its free
Help not Hospital service which supports vulnerableadultsback home after a stay in hospital.
The service will now be available seven days a week, up
from five previously, operating out of both Queen’s and King George’s
Hospitals. Those receiving support can expect someone to accompany them home
from hospital, or meet them there when they arrive, assurance that their home
is fit to return to or support with accessing other services available as well
as information and advice.
Help with stress
As a
carer you are very likely to experience increased stress.
Carers
have many responsibilities and it is very natural that sometimes carers feel
that they cannot cope. Because
ongoing stress can lead to health problems, it is important that carers receive
support from family, friends, and the services available to them. Even if
you cope most days as a carer, there might be a time when you experience
sadness, guilt, anger, or fear. Every person reacts differently to stress.
How can I
get support?
IAPT can
help! As a carer you can join our therapy programme, where you will be able to
talk to someone for free about your experience as a carer. We offer face-to-face or
telephone sessions. You can join a group session or attend individual
appointments.
In Havering more people are making the most of urgent, same
day GP appointments which are making it easier for patients to see a local
doctor.
More than 17,500 late evening and weekend GP appointments
have been made available since the service began, with appointments also
available on Bank Holidays. Should you need to be seen urgently by a doctor
there are always appointments available, just call 020 3770 1888 to book.
The appointments are available between 6.30pm and 10pm
Monday to Friday, noon and 5pm on Saturday and noon and 4pm on Sunday for
anyone registered with a GP in Havering.
Alternatively they
can be booked in through their own GP or via NHS 111. The two GP hubs which
provide the urgent appointments are at North Street Medical Care
in Romford and Rosewood Medical
Centre in Hornchurch.
Social Isolation
The Social Isolation Pilot Project is working with 100
older adults who live in the community and who feel socially isolated.
To be eligible for the project, older adults need to be
over 65 years of age and in receipt of a care package funded by Adult Social Care.
The project has three Community Navigators. The Community Navigators visit the older
adult at home and spend time finding out about the older adult’s life and
interests. The Community Navigator will
then research what is going on in the local community that may be of interest
for each individual.
The Community Navigator can provide practical advice and
support to help the older adult overcome issues around going out of their
home. This can include practical issues
relating to booking transport or attending activities with the older adult to
encourage them to meet new people.
Care Point
provides independent information and guidance to adults who require information
about care and support services available in Havering. Care Point is
funded by Havering Council to provide the service. The service is run by Family Mosaic.
The
Department of Health wish to develop a new National Carers Strategy and we have
the opportunity to influence the priorities for carers as a nation. A local
consultation was held at the May Havering Carers Forum and there are still
opportunities to influence the new strategy. Responses can be submitted via an
online survey, in writing to carers@dh.gsi.gov.uk or to The Social Care Policy Branch, The Department of Health, Richmond
House, Whitehall, SW1A. The deadline for responses is 31 July 2016.
Health 1000: Innovative new NHS initiative
GPs in Barking and Dagenham, Havering and Redbridge have
launched a new primary care organisation to help local people with the most
complex health and care needs.
Health 1000,
which is based at King
George Hospital in Goodmayes, is open to people with five or more long term
conditions or who often require lots of support from health services locally.
The patients who choose this service will switch from their current GP to the
team at Health 1000 to get their health care. If you are interested in becoming a Health 1000 patient,
you can contact
the team here or speak to your GP.
Patient Participation Groups (PPGs)
Patient Participation Groups are groups of active volunteer
patients who work in partnership with practice staff and GPs. Groups meet either face-to-face with practice
staff at agreed times or online through email networks, surveys and social
media. If you are interested in becoming a member of a Patients
Participation Group at your local GP practice, please speak to the practice
staff.
Community Dental Service
The Community
Dental Service consists of dentists, dental therapists and dental nurses
with a special interest in providing care for adults and children with special
dental care needs.
The clinic in Havering is at South
Hornchurch Health Centre, 106 South End Road, Rainham, RM13 7XR and is open
from 9.15am to 12.30pm and 1.30pm to 4.15pm
Referrals are normally made to the service by dentists,
doctors, health visitors, district nurses or other healthcare workers and/or
carers. If you think you are eligible, email the Dental Referral Team.
Adult Drug and Alcohol Service
WDP
Havering offers a free and confidential support service for individuals and
their families affected by drug and alcohol problems. The service operates from
a base in Romford and consists of specialist teams which include substance
misuse workers, doctors and nurses.

Admiral Nursing Service
Admiral Nurses are specialist dementia nurses, supported by Dementia UK, who give expert practical, clinical and emotional support to families living
with dementia to help them cope.
They are registered nurses, and have
significant experience of working with people with dementia before becoming an
Admiral Nurse. Most work for the NHS in communities, helping
people with dementia to stay at home for longer. Find out more about Admiral

'Just Once' Bereavement Service
The ‘Just Once’ service means that you can tell us
once and we will inform the government departments, local council services and
agencies that need to know if someone has died.
This year Carers’
Rights Day is on Friday 20 November. Each year Carers’ Rights Day takes
place to bring organisations across the country together, to help carers in their local
community find out about their rights and how to get the support
they are entitled to receive.
